    Effects of the addition of titanium dioxide and silaned silica nanoparticles on the color stability of a maxillofacial silicone elastomer submitted to artificial aging

    Objectives: Silicone elastomers require replacement due to degradation of mechanicalproperties and deterioration in color over time. Recently, various nanoparticles have beenincorporated into the silicones to reinforce their physical and mechanical properties. Thepurpose of this study was to evaluate the effect of nanosized particle addition on the colorstability of a commercial RTV maxillofacial silicone elastomer submitted to an accelerated aging.Materials and Methods: The nanosized particles (titanium dioxide and silaned silica) wereadded, 10% by volume, into a maxillofacial elastomer. The samples (n=10) were polymerized in anoven according to the manufacturer’s recommendations. Color measurements were performedusing digital spectrophotometry according to the CIE L*a*b* color system. The samples weresubsequently placed into an aging chamber for 168 h. The second color measurement of thesamples was performed after the aging process. Data obtained from the measurement wasrecorded and assessed by (ANOVA) (α=0.05).Results: The highest color stability was obtained with the silica and the TiO2 group samples;the lowest color stability was obtained with the control group samples in this study. However;no statistically significant difference in discoloration was observed in the ΔE values between thegroups (p > 0.05).Conclusions: The results of this pilot study revealed that the addition of nanosized TiO2and silica particles used as reinforcing agents did not cause any color degradation of anA-2000 silicone during the first 2 months. Further studies should be done on the effect of suchnanoparticles in long term aging procedures on the color stability of maxillofacial elastomers

    New generation CAD-CAM materials for implant-supported definitive frameworks fabricated by using subtractive technologies

    Innovations in digital manufacturing enabled the fabrication of implant-supported fixed dental prostheses (ISFDPs) in a wide variety of recently introduced materials. Computer-aided design and computer-aided manufacturing (CAD-CAM) milling allows the fabrication of ISFDPs with high accuracy by reducing the fabrication steps of large-span frameworks. The longevity of ISFDPs depends on the overall mechanical properties of the framework material including its fit, and the physical properties of the veneering material and its bond with the framework. This comprehensive review summarizes the recent information on millable CAD-CAM framework materials such as pre-sintered soft alloys, fiber-reinforced composite resins, PEEK, and PEKK in high-performance polymer family, and 4Y-TZP. Even though promising results have been obtained with the use of new generation millable CAD-CAM materials for ISFDPs, clinical studies are lacking and future research should focus on the overall performance of these millable materials in both static and dynamic conditions

    Imipenem-Resistant Pseudomonas aeruginosa: Risk Factors for Nosocomial Infections

    The aim of this study was to determine the risk factors for nosocomial infections of imipenem-resistant Pseudomonas aeruginosa (IRPA). A prospective case-control study was performed at a tertiary care hospital in Ankara from January to December 2004. The patients with nosocomial P. aeruginosa infection were included in the study. The features of the patients with IRPA infections were compared to those with imipenem-sensitive P. aeruginosa (ISPA) infections. Only the first isolation of P. aeruginosa was considered. Nosocomial infections were defined according to Center for Disease Control (CDC) criteria. IRPA was isolated from 75 (44.1%) patients, and ISPA was isolated from 95 (55.9%) patients during the study period. IRPA were most frequently isolated from endotracheal aspirate (19%) cultures (p=0.048), whereas ISPA were most frequently isolated from urine (28%) cultures (p=0.023). In multivariate analysis, a longer duration of hospital stay until P. aeruginosa isolation (odds ratio [OR], 1.027; 95% confidence interval [CI], 1.002-1.054, p=0.034), arterial catheter administration (OR, 2.508; 95% CI, 1.062-5.920, p=0.036), vancomycin (OR, 2.882; 95% CI, 1.130-7.349, p=0.027), piperacillin-tazobactam (OR, 6.425; 95% CI, 2.187-18.875, p=0.001), and imipenem (OR, 3.580; 95% CI, 1.252-10.245, p=0.017) treatment within the 14 days before isolation of IRPA were independently associated with imipenem resistance. It was concluded that treatment with imipenem, vancomycin and piperacillin-tazobactam were major risk factors for IRPA infections in hospitalized patients. The nosocomial occurrence of IRPA was also strongly related to the duration of hospital stay, arterial catheter administration

    Turkish Primary Care Patients’ Overviews and Attitudes About Traditional and Complementary Medicine: A Cross-Sectional Study

    Aim:Primary care physicians need to be aware of the types of traditional and complementary medicine (TCM) used by patients to be able to guide patients away from harmful approaches and to aim them toward useful or at least seeming benign approaches. This study aimed to determine the prevalence of the usage of TCM methods in Turkey and the level of knowledge and attitudes of applicants about these methods.Methods:The universe of this cross-sectional study consisted of individuals aged 18 and over who applied to the Family Health Centers and agreed to participate in the study in 12 provinces in different regions of Turkey between July and December 2016. The questionnaires were applied to the volunteer participants by the researchers using face-to-face interview techniques. The questionnaire had 25 questions.Results:80.6% of the participants benefited from TCM, and 86.4% stated they believed in the effectiveness of TCM. Women, university graduates of college who had private insurance used TCM (respectively; p=0.031; p=0.004; p=0.000), and women (83.3%) found TCM more useful than men (p=0.005). The most frequent reason for using TCM was “heard that it was useful”.Conclusion:The results of our study indicated that a large portion of Turkish primary care patients use TCM and recommend it to their relatives. Therefore, health policies and academic knowledge should be developed in this sense

    Coloring Effects of Disinfectants on Pure or Nano-TiO<sub>2</sub>-Incorporated Maxillofacial Silicone Prostheses

    Silicone elastomers play a crucial role in the field of maxillofacial prosthodontics. To maintain optimal hygiene, various disinfectants have been reported to clean silicone prostheses. Nevertheless, when selecting a disinfectant, it is important to consider not only its antimicrobial efficacy, but also its compatibility with the materials, to minimize any potential impact on the physical properties of the material surfaces. The coloring effect of such disinfectants on different types of silicone is of interest. A total of 144 silicone specimens (72 pure silicones, 72 nano-TiO2-incorporated silicones, from A-2000 and A-2006 silicones) were fabricated in this study. The spectrophotometric analysis was carried out, and the initial CIE L*a*b* color values were measured prior to disinfection. Specimens in each silicone group (with or without nano-TiO2) were subjected to a 30-h disinfection period simulating 1 year of disinfection with the following disinfectants: Control (tap water), 0.2% chlorhexidine gluconate, 4% chlorhexidine gluconate, 1% NaOCl, neutral soap, and effervescent. After the second color values were recorded, the color change (∆E*) was calculated. Significant differences were observed among the disinfectants for both the A-2000 and A-2006 silicone groups. Nano-TiO2 did not show a color protection effect in A-2000 silicone. In contrast, nano-TiO2 incorporation provided color protection against CHG 0.2%, CHG 4%, and NaOCl in A-2006 silicone. Most of the disinfectants did not show acceptable color stability over time. In pure A-2000 silicone, except for 0.2% chlorhexidine, all disinfectant groups demonstrated a color change within the acceptability threshold of 50:50% (∆E* = 3.0). On the other hand, in nano-TiO2-incorporated A-2006 silicone, only 0.2% and 4% chlorhexidine demonstrated an acceptable color change. Overall, chlorhexidine could be used as a suitable disinfectant in maxillofacial silicone prostheses

Fiber-reinforced composite (FRC) bonded fixed dental prothesis have been used as an alternative to conventional metal-based or zirconia-based fixed restorations especially among young patients. As well as the bonding system, mechanical adaptation and the retention of the fiber frame and the grooves of the pontic are fundamental for the success of this type of adhesive restoration. The esthetic solution was the building of a biologic restoration using reinforced polyethylene fibers as an interim restoration for this case

The Arab Spring not only created a domino effect between Arabic countries but also it reflected a significant influence on the financial markets all over the world. The objective of this study is to analyze the impact of the Arab Spring in Turkey Financial Market in consideration of long memory. Long memory can be defined as the persistence of the unexpected shocks on the underlying has long lasting effects. Modeling long memory in stock returns and volatility has also attracted great deal of attention from finance literature recently. Existence of long memory is determined both for the returns and volatility of the time series by using different methods. Existence of long memory can be tested by Rescaled Range Statistics (R/S), Geweke and Porter-Hudak (GPH) Model and Gaussian Semi Parametric (GSP) Method. In consequence of these tests, if the stock returns have long memory affect then respectively Fractionally Integrated Autoregressive Moving Average Model (ARFIMA) and the Fractionally Integrated Generalized Autoregressive Conditional Heteroscedasticity (FIGARCH) model are used to detect the long memory in respectively return and volatility.     Microtensile Bond Strength and Failure Type Analysis of Self-Etch Adhesive Systems on Superficial and Deep Dentin After Long-term Water Storage

    Objective: The aim of this study was to evaluate and compare the microtensile bond strengths of four current adhesive materials that contain different monomers at deep and superficial dentin. Material and Methods: Forty non-carious human third molars (n = 5) were used in the study. Specimens were divided into two main groups according to dentin thickness as superficial and deep dentin. Groups were further divided into four subgroups in terms of the adhesive systems used: Nova Compo B Plus (NCBP), Nova Compo B (NCB), Futurabond M (FB) and Clearfil S3 Bond Plus (CS3). All specimens were bonded to resin composite and stored in 37 °C water for 9-months. Teeth were sectioned into 3 x 3 mm x 1mm thick beams. Microtensile bond strength test was carried out by using a universal testing device (1 mm/min). After fracture, failure types were observed using an optical microscope and the fractured dentin surfaces were observed by a scanning electron microscopy (SEM). One-way ANOVA and Tukey tests were used to compare the adhesive system in each dentin group. An unpaired T-test was used to compare the dentin thickness in each adhesive material. Results: As compared to deep dentin, superficial dentin showed higher µTBS values in groups NCBP, FB and CS, as opposed to group NCB. Only NCB revealed higher bond strength at deep dentin layers when compared to superficial dentin. In the superficial dentin group, NCBP showed the highest bond strength value, while NCB showed the lowest bond strength. In the deep dentin group, while NCB showed the highest bond strength value, NCBP revealed the lowest bond strength value after 9-months storage. While 4-META and 10-MDP monomer combination (NCBP) resulted in the highest bond strength value among the adhesive systems in the superficial dentin group, the monomer combination (NCB) resulted in the highest bond strength in deep dentin group. Conclusions: Microtensile bond strength performance are affected by both the type of monomer in the adhesive combination and the depth of the dentin. Preventive measures must be taken in development of osteonecrosis, which can be very difficult to manage the ONJ management include ozon therapy, low level laster tratment and hyperbaric oxygen which must be combined with appropriate antibioue regime
